Werq may collect and store any personal information you enter through the Services or the Site. This may include personally identifying information, such as your name, e-mail address, phone number, office address(es), National Provider Identifier, and photographs. Werq may also request non-personally identifying information, such as the area of medicine you practice. No system can be completely secure. Therefore, Werq does not promise, and you should not expect, that such information may not be inadvertently collected and that such personal information will remain secure.
To the extent that Werq collects any personal information that constitutes “Protected Health Information” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) you provide via the Services, such information will only be used and disclosed in accordance with HIPAA and any applicable Business Associate Agreement between Werq and any Covered Entity (as defined in HIPAA) user
Like many sites on the Internet, the Site automatically collects information from your browser when you visit the Site. This information includes your IP address, your browser type and language, access times, the content of any undeleted cookies that your browser previously accepted from Werq (see “Cookies” below), and the referring website address. If you use any of Werq’s mobile applications, Werq may collect various non-identifying information, such as your device type, device properties, and IP address. In order to invite your contacts to use the Services, we may request access to your mobile device’s contacts list. By using the Services’ “Invite” feature, you will have the option to grant the Services access to your mobile device’s contacts list in order to enable to you send a text to any contact(s) you invite from your phone number. You acknowledge and agree that you are responsible for any charges that apply to communications sent from your mobile device, and because this invitation is sent directly from your mobile device’s phone number to your contact, Werq does not have access to or control this communication. If you wish to revoke the Services’ access your mobile device’s contacts list, you can do so in your mobile device’s settings menu.
Like many sites on the Internet, when you visit the Site, Werq may assign your computer one or more cookies to facilitate access to the Site and to personalize your online experience. Through the use of a cookie, Werq also may automatically collect information about your online activity on the Site, such as the web pages you visit, the time and date of your visits, the links you click, and the searches you conduct on the Site. Werq may use this data to better understand how you interact with the Site, to monitor aggregate usage by our users and web traffic routing on the Site, and to improve the Site and our Services. Most browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies. Werq employs or engages other companies and individuals to perform business functions on behalf of the Site and Services. These persons are provided with personal identifying information required to perform their functions, but are prohibited by contract from using the information for other purposes. Werq forbids the use of the Site and Services to individuals under the age of 18. Werq does not knowingly collect personal information from children under the age of 13.
Subject to the limitations under California Civil Code § 1798.83, if you are a California resident and have an established business relationship with us, you may ask us to provide you with a list of certain categories of personal information that we have disclosed to third parties for their direct marketing purposes during the immediately preceding calendar year. You may also request the identity of certain third parties that received your personal information from us for their direct marketing purposes during the immediately preceding calendar year.
